• Extensor tendon repairing with a rolled DTF graft and a TPF free flap as a gliding bed, 倉林孝之, 2012年10月, 口頭発表（一般）, Inaugural Congress of Asian Pacific Federation of Societies for Reconstructive Microsurgery, Singapore, 【Introduction】 A reconstruction of combined skin and tendon loss in an injury of the dorsum of the hand is a challenging problem. A gliding mechanism around tendon should be repaired to achieve optimum excursion. In this context, a temporoparietal fascia (TPF) free flap is useful to recreate a tendon gliding. For extensor tendon grafting, palmaris longs and extensor digitorum longus (foot) are commonly used as donor sites. However a deep temporal fascia (DTF) can be substituted for these donor sites. A Free TPF flap, a DTF graft and a skin graft all together can be harvested from one temporal region. 【Materials and Methods】 A 28-year-old man sustained the loss of the fifth extensor and overlying skin over the dorsum of his left hand in avulsion injury with sawing machine. The fifth extensor tendon was repaired with rolled DTF graft. The restored tendon (DTF graft) was enveloped with a TPF free flap as a gliding surface. A split-thickness skin graft harvested from temporal region was applied over the TPF flap. 【Results】 The TPF flap and the skin graft ware survived. One year postoperatively, functional range of movement of the left little finger was adequately restored. The donor sites at the temporal region were inconspicuous. 【Conclusion】 Extensor tendon repairing with a rolled DTF graft and a TPF free flap as a circumferential gliding bed can be a reasonable option in a dorsal hand injury. The donor sites confined to the hair-bearing scalp where scars tend to be inconspicuous, 国際会議
